چکیده

Background: Through knee amputation (TKA) may offer benefits over above (AKA) in patients unsuitable for below amputation. This retrospective analysis compared surgical and rehabilitation outcomes post TKA AKA. Methods: All AKA procedures recorded the Scottish Physiotherapy Amputee Research Group dataset from January 2007 to December 2017 were included analysis. aetiologies, re-amputations bilateral included. Demographic information, (ie, further surgery, survival, length of stay) limb fitting, early rehabilitation, mobility scores) using descriptive inferential statistics, including Kaplan–Meier, log-rank statistics multivariate logistic regression. Results: In total, 4,197 (3,471 initial AKA, 146 TKA, 580 below-knee or other level). Survival (p=0.809) stay (p=0.696) similar between groups, but had significantly higher rates surgery (p<0.001). Multivariable showed that who undergo at centres which perform small numbers are more likely need (p=0.048). A larger proportion these a fitted (25%) with only 12% those performing (p=0.041). Overall, 31% (n=23) 30% (n=725) (p=0.854). groups. Conclusion: High volume have better appear select not fit. Despite this, proportions did subsequently fit suggest superior potential although performed Scotland very small. Prospective randomised studies urgently needed inform clinical practice.

Rehabilitation of patients with through-knee amputation.

One hundred and one patients with through-knee amputations attending the Manchester limb-fitting centre are reviewed. Most amputations were performed for trauma or vascular disease. The interval from amputation to measurement for the first prosthesis averaged 12 weeks in cases of primary healing, and 21 weeks when healing was delayed. Artificial limbs were successfully fitted to 83%, and only 1...

Function after through-knee compared with below-knee and above-knee amputation.

Fifty-nine amputees, 24 below-knee (BK), 17 through-knee (TK) and 18 above-knee (AK) who had prosthetic replacements, were evaluated using a questionnaire which provided a quantitative and qualitative assessment scale for the prosthetic function. The ability to apply or don the prosthesis was noted in 100% of the BK, 70% of the TK and 56% of the AK amputations (p < 0.001). Daily use of the pros...

Knee disarticulation versus above-knee amputation.

If below-knee amputation is impossible, knee disarticulation should be considered before above-knee amputation, regardless of age and etiology. Knee disarticulation which leaves the femur and patella untoched offers many advantages. The surgical technique is simple and non-traumatic since no bone or muscle tissue is to be dissected. The thigh muscles are completely preserved and thus there is n...

Through Knee Amputation: Technique Modifications and Surgical Outcomes

BACKGROUND Knee disarticulations (KD) are most commonly employed following trauma or tumor resection but represent less than 2% of all lower extremity amputations performed in the United States annually. KDs provide enhanced proprioception, a long lever arm, preservation of adductor muscle insertion, decreased metabolic cost of ambulation, and an end weight-bearing stump. The role for KDs in th...
